How to Replace Wheelchair Bearings Safely

A rear wheel that feels rough, a caster that squeaks at every turn, or a wheel that no longer spins freely can make everyday mobility harder than it needs to be. Knowing how to replace wheelchair bearings can restore smoother rolling, reduce effort when pushing and help prevent further wear to tyres, forks and wheel hubs.

Bearings are small components, but they have a big job. They allow the wheel to turn around an axle with minimal friction. On a manual wheelchair, they may sit inside the rear wheel hub or caster wheel. On an electric wheelchair, caster bearings can often be serviced, while drive-wheel and motor components may require a technician. The right approach depends on the chair and where the worn bearing is located.

When should wheelchair bearings be replaced?

A bearing does not always fail suddenly. More often, it becomes noisy, develops side-to-side play or feels gritty as it turns. You may notice the wheelchair pulling slightly, needing more effort to propel, or stopping sooner than usual after a push. On castors, a worn bearing can make steering feel hesitant or cause vibration through the frame.

Before ordering parts, rule out simpler causes. Hair, thread, sand and dried mud can build up around a caster axle and make the wheel feel rough. A loose axle bolt, damaged tyre or bent fork can create similar symptoms. Remove any visible debris and check that the wheel is correctly secured. If it still feels rough when spun by hand, the bearing is a likely cause.

Replacing bearings is usually worthwhile when the wheel and hub are otherwise in good condition. If the hub is cracked, the caster fork is bent, or the wheel has suffered a hard impact, replacing the complete wheel or having the chair assessed may be the safer option.

Identify the bearing before ordering a replacement

Wheelchair bearings are not one-size-fits-all. Even wheels that look similar can use different bearing sizes, widths, seals and axle diameters. Ordering by wheelchair brand alone can lead to the wrong part, particularly if wheels have been changed or upgraded over time.

Remove the wheel first if practical and look at the side of the existing bearing. Many sealed cartridge bearings have a reference number stamped into the metal race. This code is the most reliable starting point. Take a clear photo of the marking before cleaning the bearing, as dirt and grease can make it difficult to read later.

If there is no visible code, measure three dimensions with vernier callipers: the inner diameter, which matches the axle; the outer diameter, which fits into the wheel hub; and the bearing width. Also check whether the bearing is sealed on both sides. For most wheelchair applications, sealed bearings are preferred because they better resist dust, moisture and everyday grime.

Caster wheels and rear wheels may use different bearings, even on the same wheelchair. Replace the matching pair in a wheel where possible. A new bearing on one side and a heavily worn one on the other can leave the wheel turning unevenly.

For an active or lightweight manual chair, confirm the wheel make, hub type and axle system before buying parts. For paediatric, tilt-in-space and electric wheelchairs, compatibility can be more specialised. If you are unsure, keep the old bearing, note the chair model and seek parts guidance before proceeding.

Tools and preparation for replacing wheelchair bearings

Set up on a clean, well-lit surface with enough room to place small parts in order. If you are working on rear wheels, engage the brakes and support the wheelchair so it cannot roll or tip. Never work underneath a chair that is only balanced on cushions, books or unstable furniture.

You will generally need basic hand tools suitable for the wheel fasteners, clean rags, a soft-faced mallet, and a bearing press or appropriately sized sockets. A bearing puller can make removal easier for tightly fitted bearings. A small amount of suitable grease may be needed for the axle or spacer, but do not pack grease into sealed cartridge bearings unless the bearing manufacturer specifically allows it.

Avoid using the new bearing as a striking tool or hitting through the centre race when fitting it into a hub. Force applied to the wrong part of the bearing can damage it before the wheelchair is back in use.

How to replace wheelchair bearings step by step

Remove the wheel and inspect the assembly

For a quick-release rear wheel, press the axle release button and pull the wheel straight out. For bolt-on wheels or castors, use the correct spanner or Allen key while supporting the wheel. Keep washers, spacers and axle hardware in the order they came off. A quick photo before disassembly can be very helpful during reassembly.

Spin the wheel by hand and check for roughness, noise and movement across the axle line. Inspect the axle for scoring, rust or bending. A new bearing will not solve a problem caused by a damaged axle.

Remove the old bearing carefully

Many wheelchair wheels have a bearing on each side of the hub with a centre spacer between them. Remove any dust cap, retaining clip or circlip first if fitted. Check closely, as trying to force a bearing past a retaining clip can damage the hub.

The ideal method is to use a bearing puller or press that supports the hub while pushing the bearing out evenly. If you are using a drift or socket, choose one that contacts the outer race of the bearing being removed. Work gradually around the edge rather than driving hard in one spot.

Once the first bearing is out, remove the centre spacer and then the bearing on the opposite side. Take note of the spacer orientation, especially if it has a stepped shape. Clean the hub bore with a dry, lint-free rag and inspect it for cracks, burrs or corrosion.

Fit the new bearings squarely

Position the first new bearing so it sits level with the hub opening. Use a press or socket that contacts only the outer race, then press it in slowly until it is fully seated. It should enter straight. If it starts to tilt, stop and reset it rather than forcing it.

Reinstall the centre spacer, then fit the second bearing from the other side. The spacer should sit between the inner races without being crushed or loose. This helps the bearings remain aligned when the axle is tightened.

If the wheel uses clips, seals or dust caps, refit them before reinstalling the wheel. Wipe the axle clean and apply only the lubricant specified for your chair's axle system. Excess grease attracts grit, particularly on wheels used outdoors.

Refit the wheel without overtightening

Reinstall the wheel and secure the axle or fastener according to the wheelchair manufacturer's specifications. The wheel should spin freely without side play. If it binds after tightening, do not continue using the chair until you have checked the bearing seating, spacer position and axle hardware.

For a rear wheel with a quick-release axle, check that the button returns fully and that the wheel cannot be pulled off without pressing it. For castors, ensure the fork turns freely and the wheel is centred between the fork arms.

Test the wheelchair before returning to regular use

Start with a stationary check. Lift each wheel clear of the floor where safe to do so and spin it by hand. Listen for rubbing, clicking or a scraping sound. There should be no obvious wobble at the wheel rim.

Then test the wheelchair on a smooth, level surface. A manual chair should roll straight and feel consistent from side to side. Check that wheel locks still engage correctly, as rear-wheel positioning can affect their contact with the tyre. With castors, turn through both directions and listen for noise under load.

After the first short outing, inspect the wheel again. If the hub feels unusually warm, the wheel becomes stiff or the bearing works loose, stop using the chair and reassess the installation. These signs can indicate an incorrect bearing size, a missing spacer or a bearing that has not seated properly.

When to arrange professional help

Bearing replacement is a manageable maintenance task on many manual wheelchair wheels, but it is not always a home repair. Arrange a qualified assessment if the wheelchair has powered drive wheels, a hub motor, unusual vibration, a damaged frame or fork, or a wheel that remains loose after new bearings are fitted.
